1. What the business actually does
Akebia sells two oral drugs to US dialysis patients. Auryxia (ferric citrate) is a phosphate binder that lost exclusivity in March 2025; Teva's generic was approved 2026-03-11 (10-Q, MD&A). Vafseo (vadadustat) is the only oral HIF-PH inhibitor approved in the US, launched January 2025 for anemia due to CKD in adults on dialysis, into an ESA market management sizes at about $1 billion, with roughly 290,000 patients now holding prescribing access (10-K, Item 1). The pipeline is praliciguat in FSGS and ebribafusp, in a Phase 2 basket trial with data expected 2027.
2. Why it looks mispriced, and why the edge case does not hold
The triage thesis was misunderstood-segment: consolidated revenue falling while Vafseo compounds underneath. That is factually true. Q2 2026 Vafseo was $21.3M, up 34% sequentially, with more than 10,500 active patients, up 41%, and about 1,200 prescribers (8-K 2026-08-05, EX-99.1); Auryxia fell to $25.5M from $47.2M (10-Q, MD&A). A crossover is coming.
Two facts break the trade. First, management has already told you the crossover does not produce growth. Vafseo leaves TDAPA and enters the ESRD bundle on 1 January 2027, and Akebia will price it "within the range of the price for ESAs," which are "priced significantly lower" than Vafseo today. So despite higher expected unit volume, "we expect Vafseo revenues to decrease significantly in 2027 as compared to 2026 due to the expected lower price point" (10-Q, MD&A). Auryxia keeps falling at the same time, so 2027 revenue declines on both products.
Second, the screen's balance sheet is wrong. It reports $130.8M of net cash by netting $155.5M of cash against only $24.8M of tagged long-term debt. The real obligations at 30 June 2026 are BlackRock/Kreos senior secured term loans of $55.0M maturing 2028-01-29, a CSL Vifor settlement royalty liability of $70.4M accreting at 9.9%, and a Working Capital Fund liability of $31.9M imputed at 15%, plus a $51.5M non-recourse liability from the 2021 sale of future Japan royalties (10-Q, Contractual Obligations). The first three are cash claims totalling $157.3M, so the balance sheet is net neutral, not half cash, and equity is $25.0M on $349.7M of assets.
3. Unit economics
CY2025 looked profitable only because Auryxia was still large: revenue $236.2M, operating income $23.5M, net loss $5.3M, with Auryxia $181.5M and Vafseo $45.8M (10-K, Item 7). H1 2026 revenue was $102.7M with an $11.0M operating loss and an $18.0M net loss (10-Q, MD&A). Vafseo gross margin is also flattered: Q2 Vafseo cost of goods was $1.0M, but at cost it "would have been $3.5 million," with $18.2M of reduced-cost pre-launch inventory left (10-Q, MD&A). Reported margin worsens as that runs out, just as the price resets. Q2 R&D was $14.1M and SG&A $28.2M on $49.1M of revenue, a ~$170M annual cost base before the June restructuring cut 13% of headcount.
4. Balance sheet and capital allocation
Cash and restricted cash fell $29.3M in H1 2026, to $157.3M (10-Q, Cash Flows). The two-year runway claim is explicitly "contingent upon us refinancing our outstanding debt under our senior secured term loan facility to defer the payment of principal, which would otherwise commence on January 1, 2027"; without it they claim only 12 months (10-Q, Liquidity). Shares outstanding rose 4.5% to 277.1M, 3.14M went out through the ATM in H1, and in June 2026 authorized common was raised from 350M to 500M shares (10-Q, MD&A), which is preparation for dilution. Insiders were net sellers over twelve months: $86,588 of buys, all of it CEO John Butler at $1.25 on 2026-03-04, against $248,932 of sales by the CCO, CAO and CFO (Form 4s). BlackRock is both the 7.2% largest holder and manager of the Kreos funds holding the secured loan (DEF 14A; 10-Q, Indebtedness).
5. Management: said versus did
Management has guided Auryxia declines and delivered them (Q1 2026 call, CFO), and was candid about VALOR: after October 2025 FDA feedback they cancelled the non-dialysis trial and said they "remain cautious about a path forward" (10-K, Item 1). The clinical claim is real: VOICE hit its stopping criteria in 2,116 patients, win odds 1.16 (95% CI 1.06-1.28, p=0.0016) versus an ESA (8-K 2026-08-05, EX-99.1). What they have not given is a 2027 Vafseo revenue number.
6. Valuation
Base (45%): 2026 Vafseo about $90M, then a 2027 reset where volume roughly doubles but price falls into the ESA band, so Vafseo lands near $65M and Auryxia near $55M; revenue about $130M on a $195M cost base, a $70M operating loss and an equity raise. Value $0.80-$1.10. Bear (35%): DaVita adoption stalls, the post-bundle price lands at the low end, more Auryxia generics arrive; 2027 revenue near $95M and a raise near $0.50. Value $0.20-$0.40. Bull (20%): VOICE makes Vafseo standard of care and it takes 25-30% of the $1B ESA market by 2030, $60-75M of EBIT at 10x, net of dilution to ~380M shares. Value $1.80-$2.50. Weighted: about $0.95 against $0.975. Reverse DCF: at a $323M EV, discounting at 12% and funding four years of interim losses, the price implies roughly $78M of 2031 EBIT, or $310M of Vafseo revenue at a 25% margin, about a third of the entire US dialysis ESA market at ESA pricing, before the dilution needed to get there.
7. Catalysts and timeline
VOCAL top-line late 2026; term loan refinancing before amortization begins 1 January 2027; VOICE top-line early 2027; the first post-TDAPA quarter, reported around May 2027, revealing the realized bundle price; ebribafusp basket data 2027.
8. Kill criteria (pre-registered, observable)
- Vafseo net product revenue below $12M in any quarter of 2027: the price cut has more than offset volume and the franchise cannot self-fund.
- The BlackRock term loan not refinanced as of the FY2026 10-K, so amortization begins against a shrinking cash balance.
- Equity issuance above 60M shares below $1.00, or a reverse split to cure the Nasdaq $1.00 minimum bid (10-K, Item 1A).
- Cash below $100M at any quarter end before 30 June 2027.
9. Verdict and summary
WATCH, conviction 2. Akebia screens at 5.9x EV/EBIT with a 25% FCF yield and apparent net cash of $131M, and all three numbers are artifacts. The EBIT and cash flow are CY2025 figures carried by Auryxia in its loss-of-exclusivity year; H1 2026 already swung to an $11.0M operating loss and $17.7M of cash burn. The net cash is a tagging error: alongside $155.5M of cash sit $157.3M of term loans and CSL Vifor royalty obligations the screen missed. The genuine positive is Vafseo, up 34% sequentially to $21.3M with 10,500 patients on therapy and a VOICE interim result showing fewer hospitalizations than an ESA. But management states in its own 10-Q that Vafseo revenue will decrease significantly in 2027 as the drug leaves TDAPA and is repriced into the ESA range, while Auryxia keeps eroding. So this is not a cheap company with a hidden growing segment; it is a sub-dollar stock financing a real but unproven franchise through a revenue trough that needs a refinancing and an equity raise, and a weighted $0.95 against $0.975 gives no margin of safety. Revisit once the first post-bundle quarter reveals the realized Vafseo price and the term loan is refinanced, both inside nine months.
